What is a Nursing Home Transition Program?

Any type of change is hard. We understand that and know that for older adults who have experienced an injury or illness, it can be even more challenging. Nursing Home Transition Programs assist with transitioning individuals from nursing home care back into the community.

How does the Nursing Home Transition Program work in North Georgia?

Legacy Link’s trained transition coordinators meet with residents and families in the nursing home to assess their options/barriers and develop a care plan for successful re-entry to community living.

 

Eligibility criteria for non-Medicaid recipients includes:
  • Must be 55 years or older
  • No Medicaid Requirement
  • Must be in the Nursing Home for 30 days
Eligibility criteria for Medicaid recipients (Money Follows the Person Program):
  • No age requirement
  • Must have Nursing Home Medicaid and either an approved Medicaid Waiver (CCSP/ SOURCE) or SSI Medicaid
  • Must be in the Nursing Home for 60 days
